Medical and other personal information that may specifically identify an individual may be protected by the Privacy Rule of the Administrative Simplification provision of the Health Insurance Portability and Accountability Act of 1996 (‘HIPAA’). We must obtain protected personal information from every client and share that information with life insurance companies and other entities that assist with the underwriting process. In order for us to obtain such information, you must submit a completed HIPAA Form to us for each client when you submit the case for review and/or processing.
